I understand that martial arts training involves physical activity, close personal contact, fitness work, partner drills, pad work, striking, kicking, blocking, falling, self-defence practice, stretching, and other training activities that may carry risk of injury.
I understand those risks may include bruising, sprains, strains, accidental contact, falls, aggravation of existing injuries, fatigue, dizziness, or other injury. I choose for the participant to take part with those ordinary training risks understood.
I confirm that the participant is physically able to take part to the best of my knowledge. I will tell the dojo before training about any medical condition, injury, medication, allergy, disability, behavioural need, trauma consideration, or other safety matter that may affect participation.
I understand instructors may adapt, restrict, pause, or stop participation where they reasonably believe it is needed for safety, suitability, supervision, or class management.
I agree that the participant will follow instructor directions, train respectfully, use equipment appropriately, and stop training immediately if asked or if they feel unwell, injured, unsafe, or unable to continue.
I consent to reasonable first aid being provided if required, and to emergency services being contacted where staff reasonably believe it is necessary. I understand that emergency contact and medical information must be kept current.
For participants under 18, I confirm I am the parent, guardian, carer, or authorised representative and can provide this consent. For adult participants, I confirm I am providing this consent for myself.
I understand this waiver does not exclude rights that cannot legally be excluded, and it does not excuse reckless, unlawful, or intentionally harmful conduct. It records informed consent for ordinary training risks and safety management.
